Desenlaces clínicos en pacientes con cardiomiopatía amiloide por transtiretina: Experiencia del primer registro institucional de un centro en Colombia

Resumen

Introducción: La cardiomiopatía amiloide por transtiretina (CM-ATTR) es una enfermedad infiltrativa causada por el depósito de proteínas extracelulares, considerada una causa importante y subestimada de insuficiencia cardíaca en personas mayores de 60 años. Existen dos tipos de amiloidosis: hereditaria (ATTRh) y de tipo salvaje (ATTRwt). A pesar de los avances en imagenología, su diagnóstico y atención siguen siendo un reto. La necesidad de una identificación temprana para iniciar estrategias terapéuticas motivó la implementación de un registro institucional en la región. Objetivo: Describir la experiencia en el desarrollo e implementación de un registro institucional para la caracterización clínica, demográfica, genética y de desenlaces de pacientes con CM-ATTR. Métodos: Estudio descriptivo de intención analítica de cohorte mixta: histórica de registros clínicos 2015-2020 (pacientes ≥ 60 años, septum ≥ 12 mm y 1 bandera roja) y concurrente 2021-2024 (pacientes sospechosos evaluados con algoritmo y pruebas genéticas para diferenciar ATTRh vs. ATTRwt). Se realizó una campaña de búsqueda activa y un enfoque multidisciplinario para registrar datos demográficos, clínicos, genéticos y desenlaces: calidad de vida (EQ-5D-5L) y supervivencia. Resultados: Cohorte mixta de 16 pacientes, de los cuales el 64.7% eran hombres, con una media de edad de 77.1 años (DE 5.85). El fenotipo presente fue de predominio cardíaco por falla cardíaca (100%), hipertensión (75%) y fibrilación auricular (56.3%). El 75% tenía síndrome del túnel carpiano. Genéticamente se encontró que el 56.3% eran ATTRh (todas por mutación V142I) y el 43.7% eran ATTRwt. El valor índice (EQ-5D) fue de 0.89 y el 33.3% refirió un estado de salud óptimo. La mortalidad fue del 18.75%, con una tasa de supervivencia del 83% luego de 40 meses. Conclusión: La implementación de un registro institucional de CM-ATTR permitió caracterizar el curso clínico de los pacientes en esta institución, destacando hallazgos relevantes en esta cohorte como la alta prevalencia de síndrome del túnel del carpo, el predominio del fenotipo cardíaco y la mutación V142I. A pesar de las limitaciones y barreras de acceso, este registro es pionero en la caracterización clínica y la descripción de la calidad de vida en pacientes con ATTR.
